- 2 Prophy (cleanings) – Periodontal procedures at additional cost
- 1 full series of X-rays (Cone-beam and Pano Extra)
- Implant Consult and Smile Consult
- Take home whitening kits/trays+monitorization-for Adults or a 1 surface filling
- Up to 8 sealants per child

25% off All regularly provided services**
- Applies only to procedures done in office
- Does not apply to referrals to specialist outside of the office-not valid with any other plan
